Fit for Work / Sick Note Requests

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill.

If you’re off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work – MED3) from a healthcare professional. Fit notes are sometimes referred to as medical statements or a doctor’s note.

If your employer asks for evidence for the prior 7 days of sickness, a fee may be charged for this. Your employer should pay the fee.

You can request a fit note from our practice via:

• Via the NHS app (Services –> Contact your GP for a document or Update –> Continue)
• By filling out an Accurx Request (Admin –> Fit Note)
• Calling our practice

You do not need to call us to be allowed to return to work, your own word to your employer is enough.
